Involvement Form.  This form is used to indicate the small group project, the small group Liturgy and the 3 individiual projects you completed for each year of Confirmation. The first should be turned into the Confirmation office by the last small group session of the first year and the second by the second year retreat.

Sponsor Form.  This form is completed at the beginning of the program when you choose a sponsor. The Arch-diocesan guidelines are indicated on the form.
